Physicians and Clinics - Due to overwhelming demand for services, a new restrictive referral policy was implemented effective 12/9/2024. Very few referrals received on or after 12/9/2024 are being accepted. Referrals FAXed to 844-454-8351. Include a clear referral question with recent records along with the usual demographic information.
NEW Patients - Physician Referral is REQUIRED. No self referrals are accepted. Also see comment above regarding current restrictive NEW referral process
Refills and Pharmacies - Refill requests - SureScripts preferred, or FAX to 844-454-8351 or 844-802-7557.
Health Plans: - Each individual actual policy will be verified for eligibility. It is best to have an image of your insurance card on file with us so we can obtain the most accurate and current eligibility verification. Patients with HMO plans are responsible themselves to verify any referral requirements imposed by the HMO have been completed, for example, a referral from your PCP "on file at the HMO", not just sent to our office.

Tricare East (prior to 1/1/2025) - CNSA Neurology was "In Network" for many years out of respect for our military families
Tricare TriWest (Arkansas Tricare beneficiaries were transferred to TriWest from Tricare East effective 1/1/2025). CNSA Neurology submitted our contract application to TriWest September 2024. As of today (5/15/2025) , we are STILL waiting for TriWest to contract and credential CNSA Neurology so currently we are out-of-network. We have contacted our Congressman, US Senator and Arkansas Medical Society regarding this matter. TriWest refuses to give CNSA Neurology any feedback on status. Patients are encouraged to contact TriWest and discuss this matter directly with TriWest. Patients are also encourage to contact the Arkansas Congressional delegation regarding this matter.
Tricare for Life Medicare Supplement - In Network
VA CCN - After many years of refusing to allow CNSA Neurology to submit an application saying "we have all the neurologists we need", in January 2025, upon our annual inquiry, VA CCN (Optum) has agreed they need more neurologists and we submitted an application. After several months, we received a proposed contract that needed extensive corrections. These corrections have been completed and CNSA Neurology has submitted a signed contract for VA CCN consideration. As of today (5/15/2025), we have not received a countersigned contract in return nor any further information as to what will be needed before we can start receiving VA CCN patients. Patients are encouraged to contact VA CCN (Optum) directly to discuss with them directly if they have concerns about this extended process.
If you wish to have our practice consider your health plan, the best method is to contact your health plan and ask them to contact us and make a proposal for our consideration.
Payment Methods - for "patient responsible" amounts we accept Cash, Checks and major Credit Cards. Credit Card payments can be made on the Patient Portal. or at the office. Check payments should be mailed to the payment address on your statement, but can also be accepted at the office. It is best to bring Cash Payments to the office.
